Job summary

Harbor Health ESP in Brockton seeks a Housing Support & Resource Specialist to help participants navigate housing, access services, and coordinate with care teams. You will assist with housing applications, referrals, and social programming to improve health outcomes.

You will collaborate with ESP staff, partner housing organizations, and care teams to deliver comprehensive support for residents across Brockton and Mattapan sites, including bilingual interactions where needed.

Qualifications

• Bachelor’s degree in Social Work (preferred) or related field; may substitute 4 years of related experience for the degree.
• Licensed Social Worker (LSW) preferred
• Current BLS (Basic Life Support) certification required
• 2 years of professional experience in community service-related field

Responsibilities

• Establishes trusting relationships with ESP participants and their caregivers; provides general support and advocates for health needs.
• Identify barriers to stable housing and basic needs; refer to local, state and federal programs.
• Assist participants with documents and applications for housing, SNAP, utilities, MassHealth, etc.
• Conduct outreach in homes or facilities; accompany participants to housing-related appointments.
• Coordinate with ESP staff to ensure integrated, coordinated care.
