Job summary

Harbor Health's Elder Service Plan in Brockton, MA, is seeking a Housing Support & Resource Specialist to assist participants and caregivers in navigating housing resources and social services.

The HSRS collaborates with care teams and housing partners to promote coordinated care, address barriers to stable housing, and coordinate programming for residents in Brockton and Mattapan.

Qualifications

• Bachelor’s degree in Social Work or related field (preferred) or substitute 4 years experience.
• Licensed Social Worker (LSW) preferred.
• Current BLS certification required.
• 2 years of professional experience in community service.
• 1 year in housing or affordable housing strongly preferred.
• 1 year experience with frail elder populations preferred.
• Proficient with Word, Excel, Outlook and Teams.
• Bilingual desirable: English/Spanish, Haitian Creole, Russian, or Portuguese.

Responsibilities

• Establishes trusting relationships with participants and caregivers; provides general support and advocates for health needs.
• Identifies barriers to stable housing and refers to local, state and federal programs.
• Assists participants in obtaining documents and completing applications for housing, SNAP, utility assistance, MassHealth redeterminations, etc.

• Develops and executes social and wellness programming with housing organizations.
• Manages housing applications and coordinates meetings with housing partners and ESP Marketing & Enrollment.
• Records care management information in the EMR within 24 hours.

Harbor Health operates two Elder Service Plan Programs of All-Inclusive Care for the Elderly, a program that allows frail elders to live in their homes and communities. Our Elder Service Plan has two medical and day centers in Mattapan and Brockton, MA and cares for more than 550 participants. We are currently seeking a talented Housing Support & Resource Specialist to join our team at the Elder Service Plan in Brockton.

Role

The Elder Service Plan (ESP) Housing Support & Resource Specialist (HSRS) will be responsible for helping participants and their caregivers/families to navigate and access community services, housing resources and other programs and resources. The HSRS works with social workers, ESP Interdisciplinary care Teams (IDTs), and partnering housing organizations through an integrated approach to promote, maintain, and improve the health of participants by addressing their social determinants of health.

In addition to working directly with participants to address individual and family needs, this position includes coordination with Marketing & Enrollment and other ESP staff, along with partnering housing organizations to develop and execute social and wellness programming for existing and prospective ESP participants who are residents within those organizations.

Direct Service

• Establishes trusting relationships with ESP participants and their caregivers/families; provides general support; advocates for individual and community health needs.
• Works with ESP participants, including those housed with community housing partners, to identify barriers disrupting stable housing and other basic needs, such as food and financial insecurity, transportation, etc.; provides support and referrals to local, state and federal agencies and programs to address those barriers.
• Assists participants in obtaining documents and completing applications for services and programming, including housing, SNAP, utility assistance, financial aid, MassHealth redeterminations, etc.
• Conducts outreach to participants in their homes and/or temporary care facilities; may accompany participants to some housing-related appointments.
• Works closely with ESP social workers and care team to help ensure that clients have comprehensive and coordinated care.

Social Wellness and Programing

• Coordinates with Marketing & Enrollment and other ESP staff, along with targeted partnering housing organizations, to develop and execute social and wellness programming for existing and prospective ESP participants who are residents within those organizations.

Housing Support

• Develops and maintains on-going professional relationships with key staff at housing partner organizations in coordination with Marketing & Enrollment and other ESP staff.
• Manages ESP current and prospective participants’ housing applications and facilitates regular meetings with housing partners and ESP Marketing & Enrollment to review application status list.
• Regularly relays information to ESP Social Workers and IDT regarding participant application status updates, as well as general resources, housing vacancies, waitlists, application processes, etc.

General Duties

• Exhibits excellent working relationships with clients, caregivers and staff, effectively communicating HHSI’s mission.
• Attends regular staff meetings, trainings and other meetings as requested.
• Records client care management information in the Electronic Medical Record (EMR) no later than 24 hours after contact.

Requirements

• Bachelor’s degree in Social Work (preferred) or related field; may substitute 4 years of related experience for the Bachelor’s degree
• Licensed Social Worker (LSW) preferred
• Current BLS (Basic Life Support for Healthcare Providers) Certification required
• 2 years of professional experience working in community service-related field
• 1 year of experience working in low-income/affordable housing-related field strongly preferred
• 1 year of experience working with a frail elder population (paid or volunteer experience) preferred
• Knowledge of existing human service systems, area housing resources and social services preferred
• Basic skills in effectively using Microsoft Office 365 applications: specifically, Word, Excel, Outlook and Teams
• Critical thinking skills to problem-solve individually and in a team setting
• Bilingual desired: English /Spanish, Haitian Creole, Russian, and/or Portuguese
• Ability to work independently and cooperatively as part of a multidisciplinary team
• Ability to prioritize, organize and schedule busy workdays
• Intermediate business writing skills
• Must be able to travel to each housing site within the Mattapan and Brockton territories and Harbor Health ESP locations in a timely manner
