Qualifications

• Two years’ experience in Social Services or related field.
• High school diploma or GED required.
• Experience navigating care systems and housing resources.
• Knowledge of HIV/AIDS populations preferred.

Responsibilities

• Coordinate housing resources and social services for clients.
• Engage and counsel high‑risk, vulnerable clients, including those in crisis.
• Complete assessments and develop individualized care plans.
• Arrange services through linkage, referral, and navigation.
• Support appointments and health care access and adherence.
• Maintain databases and document outcomes of meetings.
