Clinical and Support Options, Inc. in Northampton, MA seeks a Housing Stabilization Case Worker to support shelter guests and tenants in Northampton and surrounding towns.

The role coordinates assessments, housing planning, and follow-up for each client while advocating for access to housing resources and community supports. The position involves developing individualized plans to promote self-sufficiency, monitoring progress, and assisting with budgeting, employment, and benefits.

Qualifications

• Bachelor's or Associate's degree in a mental health discipline or high school diploma and five years' related experience.
• Valid driver's license and reliable transportation.
• Bilingual candidates are strongly encouraged to apply.

Responsibilities

• Complete Rapid Rehousing funding applications, documentation, and client assessments.
• Provide case management and community-based support to individuals in shelter and recently housed.
• Develop individualized plans to promote self-sufficiency, housing stability, and long-term housing retention.
• Monitor client progress toward goals and maintain accurate, up-to-date records.
• Assist clients with budgeting, employment, housing applications, benefits, transportation, and medical or behavioral health needs.
• Coordinate referrals to community resources, including mental health, substance use treatment, crisis services, and other support programs.
• Collaborate with landlords, service providers, probation/parole, and community partners to support client stabilization.
• Support rent management, eviction prevention, and plans for maintaining permanent housing.
• Advocate for clients in navigating public benefits, housing resources, and other social services.
• Coordinate with CES, identify available housing units, build landlord relationships, and participate in case conferences as needed.
• Work collaboratively with internal teams and external partners to ensure consistent, effective service delivery.
