Job summary

Victory Programs, Inc. seeks a Low Threshold Housing Case Manager to coordinate case management services for Willows residents in Boston. The role focuses on housing search, advocacy, and stabilization, working within a harm-reduction framework to promote health outcomes and housing stability.

The position involves collaboration with program leadership, development of networks with local services, and regular documentation of client progress while maintaining confidentiality.

Qualifications

• Bachelor’s degree with 1-2 years’ experience preferred, or High school diploma with 3 or more years of client advocacy experience.
• Housing search and retention experience.
• Case management experience.
• Experience advocating for clients with government assistance agencies, housing authorities, private landlords or realtors preferred.
• Openness to harm reduction philosophy.
• Strong organizational, written, and verbal communication skills.
• CPR certification and TB screening may be obtained upon hire.

Responsibilities

• Coordinate and advocate for housing support services to clients to improve health outcomes and housing stability.
• Provide housing search, advocacy and stabilization services including eviction prevention, recertifications, accessing funding and benefits sources.
• Attend weekly meetings and supervision, with ongoing documentation in client records.
• Develop and maintain service linkages with local providers and community resources.
• Formulate individualized plans for clients with measurable and time-bound goals.
• Follow up on post-discharge outcomes and maintain contact with providers.
