Job summary

The Housing Specialist at Intermountain Centers provides direct housing support services to assist and maintain community-based housing security and stability in Tucson, AZ. Qualifications

• Bachelor’s degree in Social Work, Psychology, or related field, or equivalent experience.
• Experience in public behavioral health and housing-related support preferred.
• Knowledge of community resources and advocacy.

Responsibilities

• Provide Housing Based Case Management services to individuals with varying disability diagnoses.
• Complete comprehensive needs assessments to identify areas of global functioning and recommend services.
• Assist the member in developing an Individualized Service Plan and provide ongoing clinical services to help achieve goals.
• Collaborate with the member’s behavioral health provider, medical providers, landlords, family, and other stakeholders to support stable housing and quality of life.
• Advocate and liaise with landlords and adult programs to promote housing/income security and health and well-being.
• Provide supports for problem-solving, wellness, self-management, housing education and vocational training.
• Maintain accurate documentation in the Electronic Health Record with daily productivity.
• May transport members in personal or company vehicles.
• Provide 80 percent of treatment contacts in the community.
• May provide 24-hour crisis on-call services on a rotating basis.
• Follow policies, procedures, and controls of the organization.
• Perform other related duties as assigned.
