Job summary

St. Vincent de Paul CARES in Sarasota, FL seeks a Peer Mentor to provide outreach, pre-screening, and coaching for Rapid Rehousing program participants.

You will support clients in navigating community resources and work closely with the Case Management team to ensure eligible individuals access needed services. Responsibilities include documenting progress, assisting with discharge planning, and maintaining data reports.

Qualifications

• Speak, write and understand English.
• Possess basic computer skills.
• Experience or education in human services or homelessness is a plus.

Responsibilities

• Provide direct outreach to individuals and families experiencing homelessness.
• Pre-screen potential applicants for RRH program eligibility.
• Coordinate screening and assessment with Case Management team.
• Provide peer support, mentoring, and coaching to participants.
