Assess patients’ housing needs and develop action plans.
Assist with affordable housing, supportive housing, NYCHA, Section 8, shelter, rental-assistance, and eviction-prevention applications.
Help patients gather documents and complete applications.
Coordinate with landlords, housing agencies, legal providers, shelters, and community organizations.
Document all patient interactions, referrals, outreach, and follow-up activities accurately and promptly.
Track applications, deadlines, and patient progress.
Work closely with therapists, case managers, and medical providers.
Experience in housing services, case management, social services, or a related field preferred.
Familiarity with New York City housing resources required..
Strong communication, organization, documentation, and follow-up skills.
Ability to work compassionately with vulnerable individuals and families.
High school diploma or equivalent required
Bilingual skills are a plus
