Responsibilities

• Support participants as they transition from street to shelter to housing.
• Conduct thorough assessments to identify risks, strengths, and needs.
• Develop strength-based action plans to promote income growth and housing stability.
• Coordinate wrap-around services (mental health, legal aid, benefits, health care, housing assistance).
• Engage participants assertively to motivate participation and follow-through.
• Maintain confidentiality and professional boundaries.
