Job summary

City Care INC in Tulsa, OK seeks an organized Advocate Housing Navigator to help individuals experiencing homelessness transition into safe, affordable housing within a psychosocial rehabilitation framework.

You will develop individualized housing plans, coordinate with community partners, document progress, and provide case management, crisis intervention, and direct support in the shelter, while maintaining confidentiality.

Qualifications

• Experience in case management, recovery, or mental health services.
• Computer literate with accurate data entry.
• Valid Oklahoma driver's license, reliable transportation, and insurance.

Responsibilities

• Well-versed in community resources, developing a network of collaborative partners
• Develop housing individualized plans with guests based on their self-directed goals
• Improve guests’ outcomes by collaborating to develop, monitor, and evaluate service plans
• Provide case management assistance, resource development, crisis intervention and direct support to individuals in the shelter
• Promote housing stability for each resident by documenting progress, referrals, and contacts in the electronic record
• Educate new incoming guests about rules and policy
• Engage in weekly face-to-face meetings with guests as directed
• Attend weekly staffing and community meetings as directed
• Uphold professional confidentiality among guests, interns, externs, volunteers and staff
