City Care Inc. is seeking an organized Housing Navigator in Tulsa to support single adults in the homelessness response system.

You will help guests develop individualized housing plans, track progress, and coordinate with partners to secure safe, affordable housing. In this full-time role, you’ll provide case management, crisis intervention, and advocacy within a psychosocial rehabilitation framework, with a focus on housing-first outcomes and collaboration with local landlords and Continuum of

Qualifications

Experience in case management, recovery, or mental health services.
Strong written and verbal communication and partnership-building abilities.
Must have a current Oklahoma driver’s license, reliable transportation, and updated car insurance.
Deep belief in housing as a human right and housing-first approaches.

Responsabilités

Develop housing individualized plans with guests based on their self-directed goals.
Improve guests’ outcomes by developing, monitoring, and evaluating service plans.
Provide case management, resource development, crisis intervention and direct support to individuals in the shelter.
Promote housing stability by documenting progress, referrals, and contacts in the electronic record.
Lead and oversee Housing Navigation team and manage individual caseload.
