Job summary

Eliot Community Human Services, Inc. is seeking a Housing First Coordinator to provide person-centered case-management for unsheltered adults transitioning to housing, linking clients to community resources and supports to sustain long-term housing, financial independence and emotional well-being.

Qualifications

• Bachelor’s degree in a human services field or social work or related field.
• 2 years case management or comparable experience with homeless population or lived experience with substance use disorder, homelessness, or mental illness.
• Must possess a valid Massachusetts driver’s license and maintain a personally owned vehicle.

Responsibilities

• Work onsite at the office and in the field with flexibility to respond to needs as they arise throughout the day.
• Engage in client-centered/harm-reduction/Housing First framework.
• Forge and maintain positive working relationships with community members and coalitions.
• Advocate to ensure client interactions with support services are person-centered/strength-based/recovery oriented.
• Collaboratively develop and implement needs assessments and action plans with clients to improve housing stability.
• Develop and implement safety response plans as needed.
• Familiarity with area resources and providers is a plus.
• Understand challenges related to homelessness, substance use, mental illness and trauma.
