Job summary

PATH (People Assisting The Homeless) is seeking a Residential Coordinator for our San Jose site to support RPSS tenants with housing stability and access to community resources. The role emphasizes coordination, health and housing outcomes, and collaboration with case managers and partners.

The position requires strong data collection and reporting, adherence to PATH guidelines, and the ability to respond to housing-related needs and emergencies.

Responsibilities

• Coordinate move-in into housing units.
• Serve as liaison between tenant and case management team.
• Conduct regular unit inspections and wellness checks to ensure habitability.
• Provide assistance for issues impacting housing stability.
• Respond to housing-related emergencies during and after hours.
• Identify and secure community resources with the assigned case manager.
• Understand lease terms and educate tenants and case managers on requirements.
• Maintain client data and complete HMIS/Salesforce entries.
• Prepare outcome reports and data-supported analyses.
