Qualifications

• Two years of experience as a County Social Services Aide 3 and two years of college level coursework including 12 credits in sociology, social welfare, psychology, gerontology, criminal justice or related social sciences; or
• A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice or related social sciences; or
• Equivalent combination of experience and training with 12 credits in sociology/social welfare/psychology/gerontology/criminal justice and one year of paraprofessional case management experience.

Responsibilities

• Referral coordination by gathering information and meeting with the consumer and referral source
• Conduct initial needs assessments to understand housing and support needs
• Develop flexible housing plans addressing system navigation, emergency housing, rental assistance
• Participate in case reviews and help resolve issues through a team-based approach
• Provide guidance to case managers and ongoing support for transition to independent living
• Offer budgeting and financial skills training to consumers receiving services
