You will develop person-centered service plans, coordinate with providers, monitor services, and guide clients toward resources and benefits, ensuring links to health and safety supports while respecting choice and dignity.

Qualifications

  • Two years of college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
  • Bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
  • Equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.
  • Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.

Responsibilities

  • Plan Development: Create individualized service plans that reflect a person’s needs and preferences
  • Service Coordination: Identify and connect individuals to providers, community supports, and natural resources
  • Person-Centered Support: Guide planning discussions that highlight personal choice and control
  • Service Monitoring: Confirm that services match what is written in the individualized service plan
  • Health and Safety Oversight: Observe and review the well-being of individuals receiving supports
  • Resource Linking: Direct individuals to benefits, services, and helpful information
