Job summary

LifeWorks of Austin is seeking a dedicated Case Manager to join our Permanent Supportive Housing team. You will deliver community-based case management, advocate for clients, and coordinate housing navigation with healthcare partners to promote housing stability.

The role involves data entry, documenting progress, and ensuring program compliance while empowering clients to reach self-defined goals within their local community networks.

Qualifications

• Requires a Bachelor’s Degree in social or behavior sciences or administrative field.
• Minimum 2 years related experience is required.
• Proficient with HMIS, Microsoft Office, and Outlook; data entry skills essential.

Responsibilities

• Provide client-centered, healing-centered case management to individuals facing homelessness.
• Identify client strengths to inform service planning and goals.
• Collaborate with partners to coordinate housing, health, and support services.
• Assist with daily needs, life skills, budgeting, and transportation.
• Maintain timely case records, assessments, and incident reports.
