Job summary

Family Promise of Hawaiʻi is seeking a Case Manager to transition families from homelessness to permanent housing using a housing-first, trauma-informed approach.

The role covers intake, housing stability planning, employment support, and community resource coordination, reporting to the Homeless Services Manager. A 4-year degree in human services or equivalent experience is required, with preference for SW master’s. Vaccination and TB clearance are noted in policy.

• 4-year degree in human services field or equivalent experience.
• Prior professional experience working in the human services field.
• Self-motivated and passionate about Family Promise of Hawaiʻi.
• Empathetic and able to empower families’ choices.
• Detail-oriented, with solid communication and presentation skills.
• Team player who works well with a variety of personalities.
• Able to accept feedback and adaptable to change.
• Flexible with work schedule.
• Valid driver’s license and reliable transportation.

Responsibilities

• Intake: Receive referrals and promptly follow up with families.
• Lead intake process: conduct intake interviews, determine eligibility, and orient new families.
• Communicate with staff and partners about family information.
• Housing and Economic Support: create housing stability plans.
• Facilitate case management sessions focusing on employment, budgeting, and barriers to housing.
• Help families find housing by liaising with landlords.
• Assist with job applications and interview preparation, budgeting, and documentation.
• Provide crisis support to families and volunteers.
• Follow up on applications submitted.
• Resources and Referrals: connect families with community resources.
• Advocate for families with community service agencies.
• Coordinate with partner organizations and maintain resource lists.
• Maintain case files and input data into databases.
• Participate in phone intake and on-call system.
• Complete exit procedures and exit surveys.
• Additional responsibilities as needed.
