Job description

Family Promise of Hawaiʻi is a 501(c)3 nonprofit working to prevent and end homelessness for families with children. Our mission is to help homeless and low-income families in Hawaiʻi achieve sustainable independence by mobilizing existing community resources and support. We provide emergency shelter, rental assistance, prevention/diversion services, and holistic case management.

The Case Manager quickly transitions families from homelessness to permanent housing, following a housing‑first, trauma‑informed approach. The Case Manager also prevents and diverts families from becoming homeless. The Case Manager assists families in overcoming barriers to sustainable independence, including finding and securing employment, increasing earning potential, maintaining housing, and accessing public benefits. The Case Manager reports to the Family Services Manager.

Essential Job Functions
Intake
  • Receive referrals from Family Promise of Hawaiʻi’s phone line and community partners, and promptly follow up with families.
  • Lead the intake process for all potential families: Conduct intake interviews, including talking with referral source(s)/partners, determine eligibility, assess appropriate level of assistance needed, and conduct program orientations for new families.
  • Communicate with other staff and partners about any necessary family information.
Housing and Economic Support
  • Work alongside families to create a housing stability plan.
  • Facilitate case management sessions with families to discuss employment, budgeting, and other barriers to stable housing.
  • Help families find housing by liaising with landlords and getting families ready as needed.
  • Assist with job applications and interview preparation, budgeting/financial coaching, and completing necessary documentation and applications.
  • Provide crisis support to families and volunteers as needed.
  • Follow up on applications that families submit.
Resources and Referrals
  • Connect families with any necessary community resources, such as child care, counseling, and educational programs.
  • As needed, work with other staff to deliver resources, referrals, and workshops related to employment, financial literacy and budgeting, housing plans, and educational planning.
  • Serve as an advocate for families with community service agencies.
Administration
  • Maintain case file records for every family, including documenting all case management activities and tracking services provided.
  • Input family data into organizational databases.
  • Participate in the organization’s phone intake and on‑call system.
  • Complete exit procedure including exit forms and exit survey.
  • Additional responsibilities as needed.
Required Qualifications & Experience
  • 4-year degree in human services field or equivalent experience.
  • Prior professional experience working in the human services field.
  • Self‑motivated and passionate about the mission of Family Promise of Hawaiʻi.
  • Empathetic and able to empower families’ choices.
  • Detail‑oriented, with solid communication and presentation skills.
  • Team player who works well with a variety of personalities.
  • Able to accept feedback and adaptable to change.
  • Flexible with work schedule.
  • Valid driver’s license and reliable transportation.
Preferred Qualifications & Experience
  • Fluent in another language, particularly Pacific Islander languages.
  • 4-year degree or master’s degree in social work.

Compensation and Benefits

The salary for this full‑time position is $55,000 – $60,000.
