Job summary

Father Joe’s Villages is hiring a Case Manager I for its Rapid Re-Housing program. You will provide hands-on support to individuals, families, and TAY as they move into stable housing, creating personalized plans and linking clients to resources.

Responsibilities include managing a caseload of up to 23 households, conducting regular meetings and home visits, advocating with landlords, and maintaining HIPAA-compliant documentation across CSTAR; travel throughout San Diego County is required.

Qualifications

• Bachelor’s degree or equivalent in social services field.
• Two years of full-time experience in case management or related setting.
• Knowledge of HIPAA standards and compliance.

Responsibilities

• Manage a caseload of up to 23 households with diverse clients.
• Conduct regular meetings and monthly home visits to assess needs and housing stability.
• Develop solution-focused case plans to promote self-sufficiency and housing retention.
• Connect clients to rental assistance and supportive services.
• Advocate with landlords, property management, and service providers to resolve housing concerns.
• Travel throughout San Diego County to support client engagement and access to services.
• Maintain accurate CSTAR documentation in compliance with HIPAA and program requirements.
• Follow program policies and HIPAA standards.
