Job summary

Penny Lane Centers is hiring a Case Manager for the Permanent Supportive Housing (PSH) program in Lancaster, CA. You will provide direct case management to a caseload of 20–40 residents, helping them access benefits, housing, and health services, both in the office and in residents’ apartments.

The role requires flexibility, teamwork, and the ability to handle multiple tasks under pressure. A Bachelor's degree in a related field is preferred, with at least 2 years of experience; bilingual

Qualifications

• Minimum 2 years of experience working with vulnerable populations and at-risk youth in social services.
• Bilingual Spanish preferred (spoken and written).
• Valid California driver’s license with clean driving record and access to a vehicle.

Responsibilities

• Provide direct case management to a caseload of 20–40 individuals/families.
• Assist participants in obtaining benefits, housing linkages, substance use treatment, and mental health/health services.
• Develop housing stabilization plans and strategies to promote self-sufficiency and safety.
• Meet with residents in office and in apartments; conduct apartment checks for safety and maintenance.
