Job summary

WestCare Foundation, based in Fresno, California, is seeking a full-time Case Manager to provide housing services to HOPWA-eligible individuals and support their transition to self-sufficiency. You will coordinate, monitor, and deliver housing-related services and build relationships with property owners and service providers.

The role requires strong communication, teamwork, and confidentiality, with a focus on housing stability and client-centered planning.

Qualifications

• Experience in housing services and case management.
• Ability to maintain confidentiality and handle sensitive information.
• Ability to work with diverse populations and collaborate with partners.

Responsibilities

• Oversight of day-to-day operations of the program.
• Build and maintain relationships with property owners/managers to secure units for leasing.
• Meet with each client weekly to support Individualized Plans.
• Intake and assessment of participants to determine eligibility and services.
• Make referrals and appointments with appropriate agencies.
• Maintain client files according to policy and contract requirements.
• Knowledge of HMIS and clinical data systems.
• Provide regular reports as requested.
• Assist with data collection and quality improvement reporting.
• Coordinate services across multiple agencies.
• Attend meetings with referring agencies.
• Perform other duties as assigned.
