Job summary

St. Vincent de Paul of Baltimore is seeking a full-time Case Manager at the SVDP Eastside Shelter in Rosedale, MD.

You will provide high quality case management services to families, develop service plans focused on housing stability, and coordinate supports to help residents transition from shelter to permanent housing. The role requires a bachelor’s or associate degree with relevant homeless-services experience, strong communication, empathy, and organization, and willingness to work flexible

Qualifications

• Bachelor’s Degree with 2 years’ experience working with persons who are homeless, at-risk for homelessness or those with a low socioeconomic status, required.
• Associate’s Degree with at least 4 years’ experience working with persons who are homeless, at-risk for homelessness or those with a low socioeconomic status, will be considered.
• Willing and able to work a flexible schedule, weekends, and evenings; if applicable.
• Knowledge of and respect for confidentiality.
• Having the ability to have empathy to support vulnerable populations.
• Motivational to encourage clients to follow their service plan and engage in service delivery.
• Strong verbal and written communication skills to work collaboratively with internal and external stakeholders.
• Critical thinking and problem solving to determine the best service plan for each client after assessing clients, analyzing notes from health care and social workers.
• Administrative organization to manage several different cases at once.
• Computer literacy to maintain and manage case records.

Responsibilities

• Develops with each client a detailed Service Plan centered on goals that assist in maintaining stable housing.
• Ensures the completion of goals documented within the ISP.
• Ensures timely and thorough case notes within documentation systems.
• Provides individual life skills support and addresses barriers to success.
• Provides a minimum of two case management sessions each month.
• Monitors and links participants to health, mental health, education, employment, housing and benefits.
• Collects monthly statistics and reports to Director.
• Complete assessments and person-centered planning to develop short- and long-term goals.
• Identify community resources and advocate to reduce barriers.
• Collaborate with internal and external stakeholders for coordinated service delivery.
