Job summary

The Shelter Association of Washtenaw County (SAWC) in Ann Arbor is seeking a Housing Case Manager to engage clients, assess needs, and create person-centered housing plans. You will provide referrals, support, and follow-up, and manage a caseload while coordinating with community resources.

Ideal candidates hold a Bachelor's or Associate's degree in human services, have strong communication skills, and can navigate diverse populations with empathy and professionalism.

Qualifications

• Bachelor's or Associate's degree in human services or related field is required.
• Excellent verbal and written communication skills are essential.
• Willingness to adhere to the NASW Code of Ethics and demonstrate diversity sensitivity.
• Ability to multitask and manage competing priorities in a fast-paced environment.
• Computer skills with proficiency in Microsoft Office and Internet resource location.

Responsibilities

• Assist with the orderly running of the Delonis Center as needed.
• Manage assigned caseload of active cases not primarily served in our residential program.
• Transport clients as needed; a personal vehicle is likely required.
• Meet with clients in the community to assess needs and progress.
• Maintain accurate client records and HMIS data entries per funding guidelines.
• Support other case managers and participate in case manager meetings.
