Job summary

The Community Partnership of Southeast Missouri seeks a full-time Supportive Services Specialist at South Side Village in Cape Girardeau, MO. You will perform direct case management, advocacy, and wraparound supports to help residents achieve housing stability and personal growth.

You will assess needs, connect residents with services (employment, education, transportation, health), maintain documentation, and collaborate with housing and community teams to deliver coordinated care.

Qualifications

• Bachelor’s degree preferred; equivalent experience may be considered.
• Minimum of one year of experience in case management, housing services, or human services preferred.
• Strong interpersonal and communication skills; ability to engage residents respectfully.
• Proficiency in Microsoft Office and case management databases (Apricot experience a plus).

Responsibilities

• Provide individualized case management and support services to residents at South Side Village.
• Conduct intake assessments, develop service plans, and maintain documentation within the case management system.
• Connect residents with community resources including employment, education, transportation, childcare, and health services.
• Facilitate workshops, group meetings, and skill-building sessions on budgeting, tenant responsibilities, and self-sufficiency.
• Support residents in crisis situations and assist with referrals for emergency resources when needed.
• Monitor resident progress toward goals and update service plans accordingly.
• Work closely with CPSEMO’s housing, workforce, and community development teams to coordinate wraparound support.
• Maintain professional boundaries, confidentiality, and compliance with all agency policies and procedures.
• Participate in team meetings, trainings, and community events as required.
• Perform other duties as assigned by the Director of Housing.
