Community Counseling Center in Cape Girardeau, MO is seeking a Community Support Caseworker. The role involves delivering community support services to assigned clients within the CPRC, managing a caseload of 20–25 with weekly meetings, and working in client homes or residential centers.

Responsibilities include training clients in daily living skills, coordinating with the treatment team, referring to community resources, and thorough documentation per policy and deadlines.

Qualifications

• Bachelor's Degree or four-year combination of education and qualifying experience.
• Four years of qualifying experience acceptable in lieu of degree.
• Qualifying experience includes mental illness, substance use disorders or developmental disabilities.

Responsibilities

• Provide community support services to clients enrolled in CPRC.
• Maintain a caseload of 20–25 clients and meet weekly with them.
• Deliver services in homes, community settings, and facilities.
• Coordinate with treatment team and monitor treatment plan goals.
• Refer clients to community resources to improve functioning and supports.
• Document interactions per agency guidelines and deadlines.
