Southwest Network is seeking a housing specialist within an Assertive Community Treatment (ACT) team to provide housing support services aligned with permanent supportive housing practices.

The role includes comprehensive assessments of housing and daily living skills, ISPs development, and coordination of services in community settings, including after-hours coverage for ACT clients.

Qualifications

• Master’s or bachelor’s degree in a field related to behavioral health; alternatives with related experience are acceptable.
• Minimum one year working with seriously mentally ill adults; ACT team experience preferred.
• Minimum one year experience in behavioral health residential programs or housing-related services.
• Must have and maintain a valid Arizona driver’s license.
• Must be at least 21 years of age.

Responsabilités

• Provide housing support services within an Assertive Community Treatment (ACT) team.
• Conduct comprehensive assessments of housing, residential, and daily living skills needs.
• Lead in ISPs and service planning for adult daily living skills and housing support.
• Coordinate, monitor, and provide direct services to members in a community setting.
• Provide after-hours care, on-call coverage, and support to all members served by the ACT team.
