Job summary

Southwest Network is hiring for a housing specialist within an Assertive Community Treatment (ACT) team in Arizona. You will assess housing and daily living needs, contribute to ISP development, and provide coordinated services to adults in community settings.

The role includes after-hours care and on-call support to all members served by the ACT team, with a sign-on bonus and a competitive benefits package.

Qualifications

• Master's or bachelor's degree in behavioral health or related field
• One year related experience required
• Minimum one year working with seriously mentally ill adults
• Must have and maintain a valid Arizona driver's license
• Must be at least 21 years of age

Responsibilities

• Provide housing support services in permanent supportive housing per SAMHSA model
• Assess housing, daily living skills, and needs; plan ISPs for adults
• Coordinate, monitor, and provide services in community settings
• Support ACT team with after-hours care and on-call coverage

Education

Master's or bachelor's degree in behavioral health or related field; or a bachelor's degree in a non-related field with one year related experience; or an associate degree with one year related experience; or a high school diploma with three years related experience
