Sentara Health Plans is recruiting for a Housing Services Specialist- Remote in Glen Allen/Richmond, VA!Status: Full-Time,permanent position (40 hours)
Standard working hours: 8:30 am to 5 pm EST, M-F
Location: This position can work remotely in Glen Allen/Richmond, VA. Travel approx 1-2x a week up to a max of 1 hour driving distance in Central VA but mainly in Richmond.
Job responsibilities:
Part of the Social Determinants of Health Team (comprised of Housing Services Specialists and Social Workers)
The Housing Services Specialist assists with housing searches for members who are housing insecure or experiencing homelessness.
Work involves providing information and assistance to help members find low-income housing options and guidance to apply for subsidized housing.
Coordinates with other staff to assist members transitioning from nursing facilities to the community.
Property visits and community networking to maintain a thorough knowledge of housing and supportive resources.
Manages a caseload through coordination and timely communication with members, staff and service providers.
Duties are performed both remotely and in the field throughout the Central region of Virginia.
Education:
HS - High School Grad or Equivalent REQUIRED
Certification/Licensure:
None required
Experience:
1 - 3 years' experience in community outreach or customer service REQUIRED.
1 - 3 years' experience in assisting older adults, the elderly, and persons with disabilities to secure accessible, affordable housing REQUIRED.
1 - 3 years' experience working with low income housing providers to develop accessible and affordable housing for older adults, elderly, and persons with disabilities REQUIRED.
Knowledge of subsidized housing and landlord/tenant rights preferred
Sensitivity to cultural and socioeconomic characteristics of the Cardinal Care population
