Job summary

HealthRIGHT 360’s Prototypes Pomona Behavioral Health Bridge Housing program seeks a Specialized Case Manager to assist clients experiencing homelessness by linking to recovery services, housing navigation, and support.

You will conduct assessments, develop Service Plans, provide weekly one-on-one case management, coordinate with external agencies, and help clients transition to permanent housing. The role emphasizes trauma-informed, gender-responsive care and meticulous documentation.

Qualifications

• Bachelor’s degree in related field required.
• First Aid and CPR certifications within 30 days of employment.
• Minimum two years’ experience in supported housing environments.

Responsibilities

• Conduct needs assessments for basic physical and social needs.
• Develop and document client Service Plans and goals.
• Provide weekly one-on-one case management sessions.
• Coordinate housing navigation and external referrals.
• Document progress in electronic systems and participate in audits.
• Support and engage clients in achieving transition to permanent housing.
