Qualifications

• Bachelor's degree in Human Services or related field.
• Experience in housing coordination or related field.
• Ability to maintain client confidentiality and accurate records.

Responsibilities

• Build and maintain relationships with local partners to enhance housing opportunities.
• Participate in community meetings and advocacy to address housing issues.
• Collect, analyze, and maintain data related to housing activities using HMIS.
• Prepare quarterly and annual reports for stakeholders and funders.
• Educate clients about resources, tenant rights, and housing options.
• Monitor files for compliance with federal, state, and local regulations.
• Conduct program evaluations and recommend improvements.
• Meet with clients in their homes to assist with activities of daily living and referrals.
• Communicate with service providers and participate in treatment team meetings.
• Work as part of a team to address long-term needs of homeless individuals.
• Attend meetings and trainings as scheduled.
