Job summary

SALT Outreach Inc. is seeking a Next Step Case Manager in Orlando, FL to empower clients toward ending homelessness and housing retention. You will assess clients, create stepwise plans, and coordinate with community resources to fill gaps in housing, shelter, and employment.

Responsibilities include conducting motivational interviews, maintaining HMIS documentation, and supervising interns. Strong communication, organization, and problem-solving skills are essential.

Qualifications

• Must have a Bachelors in Social Work (BSW) or related field (Psychology, Sociology, Human Services, Criminal Justice or other related field) or Associates with at least 3 years of Case Management experience
• Excellent verbal and written communication skills
• Strong problem solving and interpersonal skills
• Organized, accurate, thorough, detail-oriented and able to monitor work for quality
• Intermediate level with MS Office Suite and Google Docs, Google sheets

Responsibilities

• Complete an assessment to identify if a candidate will benefit from case management services
• Conduct motivational interviews with clients to determine their current resources and goals, as it pertains to helping them obtain housing, shelter, and/or employment
• Assist each client with creating a plan, prioritizing their needs by turning them into steps to help each client accomplish their goals
• Empower them to pursue their goals as it pertains to obtaining housing (or employment) and hold them accountable to those goals
• Keep and monitor a log of each organization in Central Florida that supports the sheltered and unsheltered. Update the log as needed on a monthly basis
• Refer clients to other organizations to obtain services and resources we do not offer
• Documentation will be done through HMIS to support the Continuum of Care (COC) coordinated effort to end homelessness
• Assist in supervising up to two social work interns and coordinating training
