Job summary

SALT Outreach, Inc. seeks a Next Step Case Manager to empower clients toward ending homelessness and achieving housing.

You will assess needs, convert them into actionable steps, and coordinate with community resources to support housing and employment goals. You will document in HMIS, maintain client privacy, supervise up to two social work interns, and assist in training while collaborating with leadership to ensure program quality and outcomes.

Qualifications

• BSW or related field (or AA with 3+ years case management).
• Excellent verbal and written communication skills.
• Strong problem solving and interpersonal skills.
• Organized, thorough, detail-oriented, and able to monitor work for quality.
• Intermediate MS Office and Google Docs/Sheets skills.

Responsibilities

• Complete an assessment to identify if a candidate will benefit from case management services.
• Conduct motivational interviews with clients to determine resources and goals for housing, shelter, or employment.
• Assist each client with creating a plan, prioritizing needs by turning them into steps.
• Empower clients to pursue housing or employment goals and hold them accountable.
• Keep and monitor a log of Central Florida organizations and update monthly.
• Refer clients to other organizations and follow up on referrals.
• Maintain HMIS documentation, ensure privacy and confidentiality.
• Assist in supervising up to two social work interns and participate in training.
