Job summary

Qualifications

• Bachelor's degree in a mental health related field is required.
• One year mental health experience with adults with severe mental illness is required.
• Knowledge and understanding of recovery principles and recovery driven service system.

Responsibilities

• Develops and implements service plans based on client strengths and needs.
• Prepares and completes all necessary documentation, assessments, and progress notes within required timeframes.
• Assists clients in gaining funding authorization for needed services and entitlements.
• Maintains relationships with housing vendors and coordinates referrals to housing options.
• Provides on-call crisis services on a rotational basis.
• Ensures adherence to policies and procedures and documents appropriately.

Education

Bachelor's degree in mental health field

One year mental health experience with adults with severe mental illness

Knowledge and understanding of recovery principles
