Qualifications

• Bachelor’s degree in a mental health related field is required.
• One year mental health experience with adults with severe mental illness is required.
• Knowledge of recovery principles and recovery driven service system.

Responsabilités

• Develops service plans and crisis plans based on client strengths and needs.
• Completes documentation including assessments, admissions and progress notes within required timeframes.
• Assists clients in obtaining funding authorization for needed services and entitlements.
• Maintains relationships with housing vendors and coordinates referrals and leasing arrangements.
