Carolina Outreach in Raleigh, NC is seeking a Housing Specialist to deliver in-home, community-based interventions for Mental Health and Substance Use Disorder clients receiving ACT services. You will implement crisis and behavioral therapies, coordinate with the clinical team, and support families through goal attainment in a person-centered plan.

Qualifications include a Bachelor's in Human Services and a valid driver's license.

Qualifications

• Bachelor's degree in Human Services related field, required.
• An active Driver's License is required.

Responsabilités

• Provide in-home, community-based interventions to ACT clients and families.
• Maintain oversight of services within and outside the agency.
• Collaborate with clients, families, natural supports, and collateral agencies to achieve goals in the person-centered plan.
• Provide parent/guardian training to improve interactions with the client.
