Job summary

The Harris Center for Mental Health and IDD is seeking a Care Coordinator to perform clinical assessments, develop recovery plans with the treatment team, and provide services based on consumer needs in community settings and clinics.

Responsibilities include coordinating services, delivering skills training, crisis interventions, and monitoring progress toward recovery goals while adhering to Trauma Informed and Patient Centered principles.

Qualifications

• Bachelor’s degree in social/behavioral/human services is required.
• Minimum of one year experience in the mental health field, preferred.
• Valid Texas driver’s license and insurability are required; travel may be needed.

Responsibilities

• Conduct comprehensive clinical assessments to determine service needs.
• Develop personalized recovery plans with LPHA approval.
• Deliver skills training and psychosocial rehabilitation services.
• Coordinate, link, and monitor services for clients.
• Provide crisis interventions to alleviate symptoms.
• Document and ensure compliance with guidelines.
