Job summary

The Harris Center for Mental Health and IDD seeks a Care Coordinator to provide case management and coordination of care in a housing program. You will work with a multidisciplinary team including a Master Level Clinician or LPHA and LCDC, and may transport consumers using your own vehicle with mileage reimbursed.

You will perform crisis assessments, develop treatment plans with consumers, coordinate access to services, and educate families on mental health needs and treatment adherence.

Qualifications

• Bachelor’s degree (B.A.) from four-year accredited college or university with a major in a social, behavioral, or human service field, including but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, and educational psychology, required.

Responsibilities

• performing community-based crisis assessments to determine needed level of care.
• providing crisis intervention to individuals to reduce symptoms, provide education and prevent admission to a more restrictive environment.
• collaboratively creating a treatment plan with the consumer to outline treatment goals.
• providing case management activities to assist consumers in gaining and coordinating access to necessary care and services in accordance with treatment plan goals.
• monitoring consumer progress with treatment goals and linkage to services.
• educating family members so they may understand the individual’s mental health symptoms, treatment and assist with referring to community resources.
• providing information on mental health, the importance of medications and treatment compliance, and other clinical related information to individual, family, community as needed.
