Qualifications

• High School Diploma or equivalent
• Valid New York State Driver’s License preferred
• Prior experience working with mental health consumers preferred

Responsibilities

• Provides day-to-day assistance to clients in managing their affairs and housing.
• Monitors medication administration weekly and coordinates with clinical staff.
• Maintains and reviews the 3-month housing plan with clients.
• Refers tenant issues to Housing Coordinator for action or resolution with landlords.
• Attends weekly staff meetings to discuss client status with leadership and colleagues.
• Performs additional duties as assigned by supervisory staff.
