Job summary

The Salvation Army of the Twin Cities is seeking a Harvest Hills Case Manager to provide individualized, long-term case management for residents in supportive housing programs. You will develop service plans, manage records, promote community partnerships, recruit participants, and facilitate life skills and financial literacy sessions, enabling housing stability and personal growth.

• Bachelor’s degree in a relevant field; combination of education and experience may be considered.
• If the degree is in social work, state board licensure is required.
• Previous social services experience is required.
• Experience conducting long-term case management is preferred.
• Must pass background checks and have a valid driver’s license.

Responsibilities

• Case Management (50%): Provide long-term, strength-based, person-centered case management; develop and implement service plans and monitor progress.
• Participant Information Management (25%): Maintain accurate records in physical and electronic files and ensure documentation standards.
• Community and Organizational Engagement (5%): Promote Harvest Hills program and expand participant resource networks.
• Participant Recruitment and Program Promotion (5%): Conduct outreach and build referrals to recruit eligible participants.
• Education Facilitation (5%): Lead life skills, financial literacy, and personal development sessions.
• Property Management Collaboration (5%): Coordinate with property management to support tenancy.
• Seasonal and Event Collaboration (5%): Support seasonal events aligned with program goals.

Program and Position Summary

The Salvation Army of the Twin Cities is dedicated to "Doing the Most Good" by providing holistic, person-centered services that empower individuals and families. Alongside worship services and pastoral care, programs include case management, food, clothing, utility and rent assistance, housing and shelter programs, and connections to community partners.

Among these, Harvest Hills and HOPE Harbor are supportive housing programs that help individuals transition from crisis to stability through safe housing and comprehensive case management. Participants receive assistance with employment, education, life skills, and access to community and spiritual supports.

The Harvest Hills and HOPE Harbor Case Manager provides individualized case management to residents, helping them set and achieve goals related to housing stability, employment, and personal development. This position plays a key role in promoting resident self-sufficiency, stability, and successful program completion.

Wage:$21.03-$24.49 per hour, 40 hours per week

Essential Functions (approximate time spent)

• Case Management (50%): Provide long-term strength-based, person-centered, holistic case management. Develop and implement service plans, monitor progress, and support participants in achieving their goals.
• Participant Information Management (25%): Maintain accurate and timely records in both physical and electronic files. Ensure compliance with documentation standards for case notes, assessments, and updates.
• Community and Organizational Engagement (5%): Participate in internal and external initiatives to promote the Harvest Hills program and expand participant resource networks.
• Participant Recruitment and Program Promotion (5%): Conduct outreach, build referrals, and engage with the community to recruit eligible participants and raise awareness of program services.
• Education Facilitation (5%): Facilitate individual and group sessions on topics such as life skills, financial literacy, and personal development, tailoring content to participant needs.
• Property Management Collaboration (5%): Work collaboratively with property management to support successful resident tenancy.
• Seasonal and Event Collaboration (5%): Support seasonal and special events to align with program goals and community needs.

Education and Experience

• Bachelor’s degree in a relevant field. A combination of education and experience may be considered in place of a degree.
• If the degree is in social work, state board licensure is required.
• Previous social services experience is required.
• Experience conducting long-term case management is preferred.
• Must successfully pass background checks, possess a valid driver’s license, and meet MVR clearance standards.
