Qualifications

• Bachelor’s degree in the behavioral health field plus one year of relevant experience.
• Valid AZ driver’s license and motor vehicle clearance.
• Must have fingerprint clearance issued by the Arizona Department of Public Safety.
• 3.3 weeks PTO in first year with accruals after continued service.

• Intake screenings (Completing intake paperwork and referrals).
• Completing annual behavioral health assessments, including service planning and support planning.
• Facilitating CFTs and ARTs.
• Identifying gaps in care for our members.
• Completing SMI and SED evaluation.
