Job summary

Qualifications

• Three years full-time experience as Outreach Worker or Case Manager; two years may substitute for education.
• Valid CA driver's license and reliable transportation.
• Available to work evenings and weekends; able to serve at multiple locations.

Responsibilities

• Conduct mobile outreach and intake assessments for homeless individuals.
• Provide counseling, crisis intervention, and case planning.
• Assist clients with housing searches and lease signings.
• Maintain records in HMIS and prepare reports.
• Coordinate with City and service providers; act as interpreter if needed.
