Job summary

Delta-T Group Housing Case Manager in Haverhill, MA is a temporary opportunity. You will interview shelter guests after five days, conduct intakes, assess needs, and develop case plans with progress tracked through weekly meetings.

You will maintain HMIS records and coordinate referrals to housing programs and services. The role requires a college degree or high school diploma as applicable, 1–2 years of health and human services experience with homeless populations, and basic computer skills

Qualifications

• College degree or high school diploma/GED, depending on experience.
• Minimum of one to two years' experience in health and human services work with Homeless population.
• Spanish language skills encouraged to apply.
• Basic proficiency in word processing and other computer functions using Windows, Word, and Google Email and Apps.
• Must be able to obtain and maintain crisis intervention and complete any client required training.
• Individuals experienced with housing insecurity and homelessness are strongly encouraged to reply.

Responsibilities

• Interview, assess, and begin case management for shelter guests who have been there at least five days.
• Complete intakes; assess needs and eligibility for resources to address goals.
• Develop individualized case plans with goals; monitor progress via weekly meetings.
• Maintain electronic client records in HMIS and complete required documentation.
• Provide referrals and assist with applications to housing programs and services; follow up on referrals.
• Establish and maintain relationships with community partners, housing agencies, landlords to secure and maintain housing.
