Delta-T Group is seeking a Case Manager in the Boston area for immediate start. This role involves direct services across housing, risk mitigation, transportation, and skill-building, working with families and early intervention staff to deliver services per best practices.

Candidates should have an Associate's or Bachelor's degree in a related field; bilingual Spanish is preferred. Ability to pass a background check is required. Temporary-to-perm opportunity with weekly compensation.

Qualifications

• Associate's or Bachelor's degree in a related field or equivalent experience.
• Bi-lingual fluency in Spanish is preferred but not required.
• Ability to complete and pass Criminal Background Check.

Responsibilities

• Provide direct services in housing, risk mitigation, transportation, and skill building.
• Collaborate with families and early intervention staff to ensure proper services.
• Explain clinical options and document patient progress toward recovery.
• Work with patient and family to implement effective service plans.
