Job summary

Guest House in Milwaukee seeks a Case Manager for the My Home Housing Program. You will coordinate services, perform assessments, and create service plans for participants, with on-call availability for emergencies.

Qualifications

• Bachelor’s degree in Social Work, Human Services, or related field required.
• In lieu of a Bachelor's degree, an Associates degree with 1 year homeless population experience acceptable.

Responsibilities

• Coordinate and implement services for My Home Housing Program participants.
• Be available on a 24-hour on-call basis for emergencies.
• Conduct twice-monthly home visits with each client.
• Assist clients with budgeting, errands, and transportation.
• Develop service plans and connect clients to community resources.
• Maintain client paperwork and program documentation.
• Engage with landlords and collateral contacts as needed.
• Support clients with job seeking, resumes, and SSI/SSDI navigation.
