Job summary

Pathways, Inc. seeks a Housing Case Manager for the Steuben County Department of Social Services to support individuals experiencing homelessness who have used ESG or ERAP. The role focuses on housing stabilization, employment, financial literacy, and program compliance, while addressing barriers to sustainable housing.

The HCM will provide individualized support to help clients achieve housing stability and access social services, navigating resources and ensuring outcomes.

Qualifications

• Associates with 3+ years experience.
• Preferred Bachelor's degree in social work, psychology, sociology, counseling or related disciplines and two years of case management experience.
• Experience working with homelessness, mental health and substance abuse.
• Strong communication skills.
• Strong sense of boundaries.
• Valid driver s license and own a motor vehicle.

Job description

Nature of WorkSteuben County Department of Social Services Housing Case Manager (HCM) will provide support and services to individuals who are experiencing homelessness and have utilized Emergency Solutions Grant (ESG) or Emergency Rental Assistance Program (ERAP). The HCM will provide individualized and specialized support to individuals focusing on and obtaining housing. This will include but limited to employment, financial literacy, meeting program expectations, maintaining housing and assisting in barriers they may experience.

Minimum Qualifications

• Associates with 3+ years experience. Preferred Bachelors degree in social work, psychology, sociology, counseling or other related social service/science disciplines and two years of experience providing case management services.
• Experience working in services for homelessness, mental health and substance abuse.
• Strong communication skills.
• Strong sense of boundaries.
• Valid drivers license and ownership of a motor vehicle
