Job summary

Housing Works in New York City is seeking a Housing Case Manager for the Emergency & Transitional Housing Program (ETH). You will deliver direct case management, housing placement assistance, psychosocial assessments, and coordinated access to community services.

Responsibilities include managing 35–45 clients, developing Treatment and Housing Plans, assisting with IDs, benefits enrollment, and housing applications. This role requires a Bachelor's degree and strong communication.

Qualifications

  • Bachelor’s Degree required in social services, psychology, social work, or a related field.
  • Experience providing case management or direct client service.
  • Experience working with individuals involved in the criminal‑legal system, as well as those with substance use or mental health diagnoses.
  • Strong written documentation and communication skills.
  • Proficiency in Microsoft Office Suite, particularly Excel.

  • Conduct psychosocial assessments with all newly enrolled clients.
  • Conduct housing eligibility screenings to inform and develop housing plans.
  • Co‑develop robust Treatment and Housing Plans, reviewing every 30 days.
  • Meet with clients regularly in accordance with program workflow and contract deliverables.
  • Assist clients in obtaining or retrieving vital documents such as government‑issued IDs, social security cards, birth certificates, etc.
  • Assist clients with benefits and entitlements enrollment.
  • Assist clients with housing applications and scheduling apartment viewings.
  • Work with clients to facilitate family reunification.
  • Connect clients to internal and external health, wellness or other support services.
  • Escort clients to service appointments and coordinate alternative transportation services.
  • Engage in care coordination activities with internal and external providers.
  • Conduct case conference meetings with clients and interdisciplinary care teams.
  • Engage in crisis intervention as needed.
  • Maintain accurate and up‑to‑date client charts using Housing Works electronic medical records systems.
  • Adhere to program and agency quality assurance standards.
  • Participate in quality improvement activities.
  • Submit daily, weekly, monthly, and ad hoc reports in a timely manner.
  • Participate in program and agency meetings.
  • Promote Housing Works’ core values in day‑to‑day activities.
  • Perform other tasks as assigned by supervisor.

Compensation Range: $28/hr – $31.25/hr, commensurate with experience.

Benefits: Housing Works offers three comprehensive healthcare plans. Housing Covers the majority of the plan; the employee pays the remaining portion based on salary. Staff accrues PTO immediately, totaling up to 30 days in the first year. An educational benefit is available for tuition loan reimbursement, tuition costs, and textbooks.

Overview: The Housing Works Emergency & Transitional Housing Program (ETH) provides time‑limited transitional housing and support services to individuals reentering the community after incarceration or other justice system involvement. Participants receive case management, vocational training, housing advocacy, and mental health support to build skills and overcome barriers to stable housing. The Housing Case Manager delivers direct case management and housing placement assistance, including psychosocial assessments, treatment and housing plans, and connections to community‑based services.

  • Deliver culturally competent services in accordance with Housing Works policies and practices.
  • Advocate in alignment with Housing Works and the Justice Initiatives Department policies.
  • Work with clients of varied service needs and backgrounds.
  • Collaborate with other team members within the Justice Initiatives Department and across agencies.
Other Functions
  • Attend staff meetings and trainings as scheduled.
  • Respond promptly to emails, texts, and phone calls.
  • Show flexibility to work pre‑scheduled evening hours to meet client service deliverables.
Requirements

Bachelor’s Degree required in social services, psychology, social work, or a related field.

  • Experience providing case management or direct client service.
  • Experience working with individuals involved in the criminal‑legal system, as well as those with substance use or mental health diagnoses.
  • Strong written documentation and communication skills.
  • Good problem‑solving skills and ability to work autonomously.
  • Proficiency in Microsoft Office Suite, particularly Excel.
Additional Competencies
  • Preferably bilingual in Spanish (not required).
  • Excellent written and oral communication skills.
  • Technical proficiency to perform tasks related to Microsoft Office, email, and case management systems.
  • Ability to work independently and collaboratively with the team.
  • Dependability and reliability to arrive and depart timely for scheduled shifts.
