Job summary

Community Bridges, Inc. (CBI) seeks a Client Care Coordinator to manage a housing caseload and coordinate comprehensive care for individuals and families experiencing homelessness.

The role involves ensuring connections to medical homes and primary care services, working within a multidisciplinary team to address high-acuity needs. The position emphasizes coordination of care across behavioral health and medical services, with growth opportunities within CBI's continuum of care.

Qualifications

• High school diploma or GED required.
• Associate degree (or higher) in Behavioral Health preferred.
• Minimum of six months of recovery from substance use and/or mental health disorders required.
• Minimum of six months paid related work experience with behavioral/medical crisis exposure required.
• 1–3 years of full-time healthcare-related work experience preferred.

Responsibilities

• Manage housing caseload and coordinate care for individuals and families experiencing homelessness.
• Coordinate care with existing PMCH/PCP or connect to PMCH/PCP services.
• Collaborate with a multidisciplinary team including PCTs, RNs, Counselors, and Clinical Leads.
