Job summary

Community Bridges, Inc. (CBI) seeks a Clinical Coordinator to supervise Case Managers and Rehabilitation Specialists, oversee clinical quality, and mentor staff on an assigned team.

You will ensure assessments and treatment plans are developed and implemented for each patient, while maintaining productivity standards and program fidelity. The role emphasizes supervision, coordination across behavioral health services, and ensuring quality of care in a growing nonprofit healthcare provider

Qualifications

• Bachelor’s degree in behavioral health or related field.
• 1 year experience with the SMI population in multiple settings and levels of care.
• 1 year experience in a behavioral health position providing outreach and engagement activities.
• Ability to pass criminal background check for jail visitation/clearance.
• Valid Arizona Driver’s License with 39 month clear MVR.
• Behavioral Health Technician (BHT).
• CPR/First Aid; Crisis Prevention Institute (CPI).

Responsibilities

• Provide clinical and administrative supervision, oversight, training and mentoring for Case Managers and Rehabilitation Specialist on assigned teams.
• Ensure clinical quality and productivity standards are met and exceeded by direct reports.
• Ensure assessments and treatment plans are developed and implemented for each patient on the team.
