Pacific Clinics is seeking a Housing Monitor to provide client advocacy and case management under the Housing Supervisor. The role links clients to mental health and substance abuse services and coordinates supportive housing activities.

The incumbent will monitor program compliance, perform intakes, document thoroughly, and collaborate with external agencies to secure benefits. A strong, compassionate approach is essential.

Competenze

• High School Diploma or GED required and two or more years of related experience, or equivalent.
• Experience in social services or related fields preferred.
• Bilingual skill preferred for client population.
• Current CPR/First Aid certification required or willingness to obtain.
• Background check (DOJ/FBI) required.

Mansioni

• Promotes a strength-based environment to help clients achieve goals.
• Monitors client compliance with program rules and policies.
• Conducts intakes, screenings, and client move-ins/move-outs.
• Provides case management support, including living skills and benefits linkage.
• Uses de-escalation and crisis intervention techniques.
• Maintains cleanliness and sanitation of common areas and client rooms.
• Performs wellness checks and safety-related inspections.
• Completes incident reports and HMIS entries within timelines.
• Coordinates with outside agencies to access benefits and services.
