Job summary

HOM, Inc. is looking for a Housing Specialist in Arizona to manage a caseload of approximately 225–250 participants across various supportive housing programs funded by state, federal, and local agencies.

  • Proficient with Microsoft 365 apps: SharePoint, OneDrive, Teams, Planner, Outlook, Word, Excel.
  • Willing to take on other duties as assigned.

Responsibilities

  • Provide timely, excellent customer service in person, phone, or email.
  • Maintain weekly communication with participants and care teams.
  • Coordinate with HOM departments for timely service, data entry, and reporting.
  • Represent HOM in meetings with providers and external stakeholders.
  • Conduct housing program orientation and intake for eligibility.
  • Access participant data in HUD EIV, AZTECS, GUIDE, and other systems.
  • Register participants in Padmission Connect and ensure access to needed services.
  • Educate property owners/agents on program policies; coordinate RFTA and leasing.
  • Review lease, finalize subsidies (HAP/RAA), and ensure compliance.
  • Distribute Move-In Kits and document actions in software.
  • Conduct interim/annual recertifications and unit transfers.
  • Coordinate care team efforts for at-risk participants; document non-compliance actions.
  • Maintain physical and digital participant files; ensure auditable records.
